HER2陽性乳癌　即使早期小腫瘤術後仍應積極治療

47歲張小姐洗澡時突然摸到乳房附近的硬塊，就醫檢查確認為HER2陽性乳癌，腫瘤雖已有一兩公分大，但未轉移至淋巴，看似幸運，實則已經失去了標靶藥物健保給付的資格，受限於經濟能力，術後未能接受輔助性標靶藥物合併化療，兩、三年後不幸復發。

個人化治療標靶　提升HER2陽性乳癌存活機會

台北馬偕紀念醫院一般外科主治醫師林鉷彬指出，隨著基因檢測、病理化驗等技術的進步，以及標靶藥物的研發，乳癌治療已朝向個人化治療，針對乳癌的病理報告結果及分子生物學的檢驗報告，可針對每一位乳癌患者設計不同的治療方針。

舉例來說，荷爾蒙陽性的乳癌患者屬於轉移低危險群，復發率較低，或許只需接受抗荷爾蒙治療。但如果為高危險群的HER2陽性患者，預後情況較差，復發率高、死亡率高，所幸在抗HER2標靶藥物問世後，大幅降低復發風險及提高存活率。

越來越多的研究證實，早期HER2陽性乳癌即使腫瘤小於一公分，無淋巴轉移，仍有復發風險，張小姐即為典型個案，腫瘤並不大，且無淋巴轉移，術後初期情況不錯，但兩三年後仍然轉移到肝肺等器官。

早期HER2陽性乳癌　術後標靶治療降低5成復發風險

臨床證實，這類患者術後使用抗HER2標靶輔助療法一年，再搭配化療，能有效降低5成復發風險、6成死亡風險。由於HER2陽性乳癌的預後較差，所以當腫瘤大小達T1b，也就是0.5公分以上，即會建議患者術後應接受化療與標靶藥物治療，以避免遠端轉移復發。

不過受限於健保預算，目前腫瘤小、淋巴沒有轉移的HER2陽性乳癌患者，即使存在日後轉移復發的危險因子，但健保仍無法給付抗HER2標靶輔助療法，患者必須自費，不少經濟能力不佳、無力負擔的癌友只能無助地等著癌症轉移的那天到來。

林鉷彬醫師指出，門診收治不少類似張小姐個案，礙於健保給付規定，無法在第一時間選擇最適合的藥物，復發轉移的風險因而增加，建議能夠適度開放用藥規定，事實上，HER2陽性患者如能夠使用預防性標靶藥物治療，減少日後復發機率，反而能減少更多未來的醫療支出。

目前乳癌的治療武器非常多種，治療的機會亦相對提升，提醒乳癌患者不要因為害怕手術或治療可能發生的副作用，就逃避轉為尋求其他輔助治療，其實醫院都有完善的醫療照護團隊，可以在接受治療時給予及時的照顧及處理，如延誤治療，喪失治療黃金時機，導致癌細胞轉移等，反而得不償失。
